Merkle Wellness Solutions is in network with Buckeye, Ohio Medicaid, CareSource, United HealthCare (UHC), AmeriHealth and Humana. If you have one of these plans, you can book an appointment here.
Merkle Wellness Solutions is also partnered with a company called Berry Street to offer additional coverage with Anthem/BCBS, Medicare, United Health Care (UHC) and Aetna. If you have one of these plans, you can book an appointment here.
If you do not have one of the above plans, self pay clients are offered a discount from the retail rate and can schedule at this link.

If your insurance does not cover nutrition services, Merkle Wellness Solutions can provide a SuperBill for possible insurance reimbursement on request.
The cost for initial appointments is $150 with a 40% discount offered to clients who are self pay.
The cost for a follow up appointment is $120 with a 40% discount offered to clients who are self pay.
FSA/HSA are accepted as payment for services.

Initial Appointment
These appointments last 60-90 minutes. We will discuss your health history, current medical problems, nutrition history and anything else that is pertinent. From there, we will determine an appropriate nutrition intervention, set goals and create a plan for success.
Follow Up
These appointments are 45-60 minutes. We will follow up on your previous appointment, we will identify progress made along with challenges and opportunities for improvement.
We will also use these appointment to go more in depth into nutrition education that is perinent to your medical conditions or goals.
